NEW PATIENTS

Siouxland Mental Health - New Patients

WOODBURY RESIDENTS ONLY

New clients are accepted Monday-Friday 8:00 – 3:00 at Siouxland Mental Health Center, 645 Court Street. During this first visit you will complete and review paperwork with an intake person on a first-come first-serve basis. (Please do not mail paperwork)

IF YOU HAVE INSURANCE, PLEASE CALL YOUR INSURANCE COMPANY:

  • to verify that you have mental health benefits under your plan
  • to verify if Siouxland Mental Health is covered as an agency and/or which providers

 

Please bring insurance card with you.

IF YOUR INSURANCE COMPANY DOES NOT HAVE MENTAL HEALTH BENEFITS:

  • Bring a statement of proof from your insurance company
  • You can apply for financial assistance through Woodbury County (funding application below)

IF YOU DO NOT HAVE INSURANCE:

  • You can apply for financial assistance through Woodbury County (Funding application below)
  • We accept Title 19 ONLY from Iowa.

INTAKE VISIT

During your intake visit please allow ample time to review paperwork with an intake person, which will be done on a first come first serve basis.

  • Adult patient – patient must be present at intake.
  • Child patient – legal guardian must be present at intake. If child has been adopted or is in a custody situation, please bring all legal paperwork. We will make copies for our records.
  • All patients – In order to provide continuity of care, we will need to receive records for all previous treatment. Please bring names and addresses of previous providers such as psychiatrists, therapists, substance abuse services, and mental health hospitals.

Please print and complete appropriate new client paperwork.  Bring at time of intake visit.

NON-WOODBURY RESIDENTS ONLY

If you live outside of Woodbury County and have insurance, please contact your insurance company to make sure it has mental health benefits. If you do not have any insurance or you do not have mental health benefits, you will need to contact your local mental health center in your county in order to receive financial assistance.
