ADULT & PEDIATRIC INTEGRATED HEALTH HOME
An Integrated Health Home (IHH) is a team of professionals working together to provide whole person, patient-centered, coordinated care between behavioral and physical health providers to improve quality of care & outcomes for adults with a serious mental illness (SMI) and children with a serious emotional disturbance (SED).
Integrated Health Home is a program running in the Siouxland area to address whole health for the mentally ill. As part of a total health process, Integrated Health Home works to serve the mentally ill to both enhance and extend the life of people who suffer from mental issues. By addressing physical health and wellness, Integrated Health Home hopes to extend long and healthy lives of the mentally ill in the Siouxland area.

HOW DOES THIS PROGRAM WORK?
You will get a health and wellness screening. This will help your providers understand you and your unique health needs. You will learn more about your overall health.
Your health care team will focus on meeting your needs. They will share information to get a complete view of your health.
Your team will work together to:

Make sure you have access to care 24 hours a day.

ELIGIBILITY
You are eligible for the services if you are:

Enrolled in Medicaid

An Adult with a Serious Mental Illness

Schizophrenia
-
Schizoaffective Disorder
-
Bipolar Disorder
-
Major Depression
-
Other serious mental health conditions that cause significant impairment in daily functioning.
